I was there in April--came in on the train from Vienna; they checked my passport and ticket and moved on.

Regardless of where you travel or whether it is required or not, you should always purchase travelers insurance that covers medical and evacuation expenses. For example, the evacuation charges for a severe leg break that requires special seating on an airplane to get you back home for recovery starts at $20,000USD and can very easily be double that if the accident happens in a remote place or you live in a remote part of the US. I've heard horror stories of travelers being financially ruined by these charges. I never take a chance and always buy insurance. There are lots of companies that provide this kind of coverage I happen to use this one:

I was also there in April, arriving by night train from Munich, and like Chris, they only checked my passport and ticket.

Hi Donna, I'm married to a Czech national and go there every year to visit his family. Never have I been asked for proof of insurance. Just go and have fun! One side note, most stores close early on the weekends. So if you happen to get sick in Prague in the evening on a weekend, your best bet for pharmacy items is a store in the Mala Strana (Kampa Island) near the Charles bridge going towards the Castle. There are pharmacies all over Prague and in every town for daytime hours. Just look for either a green cross or a green medical scales symbol. Most folks in Prague know enough English to understand what you'll need or they will get someone who can. The smaller towns are a different story, as they are ran by usually older folks that don't know as much English. Not to worry, if you run into that situation, ask the person you are renting a room from if they can assist in interpreting.
