YOu can arrange a guided tour in advance through a travel agency, but there is really no advantage to doing so. There are registered guides at the gates of Ephesus. You can recognize them by the ID's they wear around their necks. You can (and should) negotiate price on the spot. I recommend chatting long enough to assess the guide's English language facility. We hired a wonderful elderly gentleman whose stories about the site added immensely to our enjoyment, although he had to sit down to rest periodically.
Don't have any info regarding guides, but I do have a tip. Besides wear a hat and bring water! About half-way through Ephesus, there is a modern-looking "building". Inside are the homes of the well-to-do from the past. There are some wonderful frescoes and mosaics. It costs extra to get in. I forget the exact cost, but it was about the same as the regular admission. You pay at the entrance to the "building". It's well worth the price. And it's protected from the sun, so that's a plus.
