We would like to do a one-day tour Zurich to Appenzell. Any recommendations welcome. Is it worth it?

It is certainly possible to go to Appenzell from Zurich for the day and return to Zurich.
What do you plan to do there?
As I recall, it is about a two hour train ride from Zurich to Appenzell. Four hours total travel time to spend a few hours in Appenzell isn't something we would do.
We hiked the Appenzeller area staying in berghuttes last July. The hiking is incredible and the huts are perfectly located for lunch stops and overnight stays. They are pretty empty of people mid-week also. Some of the huttes are next to working farms.
You can visit some of the best known locations in the mountains via cable car if you don't want to hike. Example - Aescher is a short walk down from a cable car:

Walk past the restaurant to the little barn. There were puppies, a couple miniature horses and a flock of sheep when we were there.
The town of Appenzell is much slower paced and relaxing than Zurich. Even if you don't hike, I'd recommend spending a couple of days to explore the area.
