Question 1: I was fully vaccinated in March of this year. According to Swiss requirements, at time of my departure, May 2022, I will no longer be considered vaccinated as it will be greater than 365 days since my second dose. This means I will need a booster correct? No where in the literature is booster specifically mentioned.
Question 2: My name on vaccine card doesn't match my passport. I chose not to officially change my name (marriage) until after the trip due to long wait times at the passport offices. My plan is to bring vaccine card and show marriage certificate. Not sure if this will work or how this plays into the Swiss covid certificate. Thoughts/experience?

It’s too soon to know what will be required in May 2022. I expect the rules will change many times between now and then. I’d start checking a month or two before you leave. But as of today, regarding the 365 days, If your second shot is great than 365 days, you would need a third shot. From what I have been reading, nothing official yet, the definition of “fully vaccinated” may soon be changing to 3 shots for Pfizer and Moderna and 2 shots for J & J.
Can you get a new ( re-issued) CDC card to match your passport name?
That may be easier than the 3 document approach ( passport, CDC card and marriage cert) at a foreign border
Joe32F, unfortunately not. The issuing authority cannot make any changes to the certificate. It was suggested to use VSafe as additional proof, but there is no way to print a card from VSafe. It was only to record side effects, etc. I found a Swiss government covid certificate based email that I will try tonight to ask about documentation. This has to have been discussed with people getting married and divorced all the time (name changes abound).

Go get a booster at a local pharmacy and have them do a new card and have them write in your first two.
That is what I did. I go by two names so I had them add AKA xxxx to mine.
