I have been to Switzerland once. I already did Lucerne, Interlaken & Lauterbrunen.
I am planning this trip for 10 days and planning to cover the following:
1) Appenzell
2) Silsersee or Gstaad
3) Lauterbrunnen area
Is there anything else I can add or replace?

When are you going? I like Zermatt and Montreux. And I can never get enough of Wengen and Murren.

Look at Rick's list of sights worth seeing in his guide, and go to the ones you haven't been to yet.
And revisit anyplace you loved.
You could do a daytrip on the Bernina Express, which would be a long day. Or spend the day on the Glacier Express, or take the Golden Pass into Montreux. Or go to Italian Switzerland/Ticino.
