In October we will be spending a week in Lucerne taking day trips using a Swiss pass. At the end of the week we will be travelling to Rome from Lucerne. My question is; can we use the Swiss pass from Lucerne to Milano Centrale and buy a Italiarail ticket to Rome.

The Swiss Pass just covers you to the border. At Lucerne train station, buy a separate, reserved ticket from the border point (usually Chiasso, sometimes Iselle) to Milan. They can also sell Milan-Rome, or you can book that leg online through Trenitalia. You do not have to get off the train at the border.
