I have the CDC card and a QR code from Smart Health Card. Is this acceptable to enter Spain? thank you

Any QR code obtained from a US source is not going to be usable in Europe. The CDC card will be fine.

What I have read is that US QR codes are not 'readable' by European entities

I thought Spain still required the "Health Control Form" to be completed: https://www.spth.gob.es/

What Melinda posted above is correct. You must get a QR code from the Spanish government portal before flying to Spain at www.spth.gob.es

Thank you for all of the responses. The Smart Health Card was provided by "Docket" which is the mobile app that N.J. uses. The EU states if you have a QR code that is accepted by the EU you do not need to fill out the forms to enter Spain. There are six of us traveling and I thought we might luck out and not have to fill out those forms. Thanks again for the information.
