If I am understanding the process correctly you cannot fill out Spain's health control form until 72 hours before arrival. Does anyone have the details that are required to fill out the form for vaccinations or where I can find a copy? I have searched for a copy of the form and haven't found anything. Any help is greatly appreciated

I just completed the form last night. Simple form. there are videos on google too that can help you fill in the details. But it took 10 mins to complete the form and got the QR code with in 15 seconds of submitting it.

Delta sent me a link to fill out the form this morning. Two days before we fly and three days before we arrive in Madrid.
No reason to fill it out early.
We fly through CDG Paris and Air France, who we will fly with from CDG to MAD, also sent me a link.
