If Spain is now requiring a test to go over, in my area we’d go to a CVS. We also have a health place that does rapid (results within 24 hours) PCR tests for a fee. When leaving Spain, there are clinics at some of the major airports. Which airport are you departing from to return to the states? There are also clinics in town. Appointments are required every place I mentioned.

Edited: As I read the requirements as of late December, no test is needed to enter Spain, but one must be fully vaccinated. One must also fill out the Spain Travel Health form either via the app or website. It will generate a QR code that must be presented upon arrival. Everything you need to know can be found on the Spain Ministry of Health website.

If you’re returning from Madrid you can use one of the Quiriónprevención clinics. There’s one in the area near the Prado at Calle de Francisco Zea, 8. There’s a clinic at the airport and other providers around town.

@cowboycork - its a general hospital in the Gracia neighborhood, its just the one location in Barcelona. In Spain, "clinica" is another name for hospital.
