We are arriving in Madrid mid-March and would like to get a SIM card for my iPhone. Is there a booth at the airport and how much would it be? Can I use this phone in Portugal as well?
Any information on type, cost, etc. would be appreciated.

Can't help you with an airport location for a mobile store.
Yes, you should be able to roam to Portugal on the same SIM. The EU has gotten rid of most roaming fees. Just ask when buying the SIM about using it in Portugal.
And make sure your phone is unlocked and will work in EUrope.

Hi Andrew,
The Canadian Federal Government mandated as of Dec. 1/17 that all phone companies have to unlock your phone at no charge so my iPhone is unlocked! It's an iPhone SE so should work in Europe...
