I saw one on Trip Advisor. Anyone taken a tour from Madrid?
I took the Toledo/Segovia day tour from Madrid and it was fantastic!! (yes, I used Trip Advisor, but the tour I selected was bilingual Spanish/English).
However, I have been to both those cities before multiple times, as well as Ávila.
The truth is that if this is your first visit anywhere, you would probably find it more worthwhile just to go to one place for a day. Your trip would be more leisurely, and you could take time to relax a bit for the day.
In Segovia you need to see the aqueduct and the Alcázar, in Ávila you should see the cathedral, St. Theresa's birthplace, and of course, the walls!

I haven’t been to Avila, and stayed a couple of nights in Segovia (so just went on my own). So I have no experience, but have recommended the Toledo tour with this company to some people who don’t want to go on their own.
Last year we took a tour from Madrid to Segovia and Ávila. We simply saw a tour advertised as we were walking and went inside and purchased tickets. We enjoyed the tour and were happy we chose it. Sorry I don’t have information as to the company but do remember that once we were in each town there was some history given by the guide and later we were able to walk about on our own. It was not rushed and was a fun day. There was time to pick up lunch.
