How hard is it to return a car in Toledo? We will rent the car in Bilbao, drive it for about ten days, and will be arriving via Segovia. Our plan is to return it upon arrival, then spend a night in Toledo, and return the next day by train to Madrid. Any recommendations for car rental companies that may simplify this, or does it not matter? Is it necessary to book train tickets right now for that trip?

First, go to autoeurope.com to see which companies offer rentals from Bilbao to Toledo. Then check where the offices are in Toledo, which is San-Francisco-hilly with the added spice of centuries-old streets.
It may well be easier to drop the car at Madrid Atocha (and surely more rental company options) and then take the 1/2 hour train ride to Toledo. BTW if you have bags, you can store them in a locker in Atocha and go to Toledo with a small overnight bag.

Of the major agencies, Enterprise is near Toledo train station, Europcar is also within the new town, and Hertz is further out of town.
Was just in Toledo (not driving) and you definitely don't want to drive in any part of the historic core (on the hill) of Toledo. Around the train station or other parts of the "flat land" looked like it would merely present the "normal" challenges of driving in any urban area in Spain.
