I have a question about filling out the form on the SPTH website. When you enter your vaccination information, do you enter that info manually or do you have to upload your state QR code or scan your paper CDC card? I'm asking because my QR code from my state has my name listed incorrectly. ( I'm trying to get it corrected before my trip. ) My paper CDC card is correct.

You enter the information - no actual upload.
The form is an attestation to the truth of the info you put in. There are (seemingly random) checks of the underlying proof, your CDC card, at the arrival airport.

We just arrived in Madrid 3 days ago. We filled out health form on the app ( handy since we had a missed connection and had to redo form for new flight) uploaded pictures of our passport and picture of our vaccination cards. Saved form to our phone app. Could also print out at home. Husbands app kept having issues with date however I could load his info onto my app and all was good. There was a health form check station as the last stop before leaving airport, after customs.
