My family and I are hoping to catch the 6:20am AVE from Madrid Atocha to Barcelona. The earliest bus departure looks to be 7:59am from ALSA. Are there any private shuttles we can hire?
If you are staying at a hotel, email them and ask if they can order you a shuttle. That is what I did when I had to leave my hotel in Paris at 5:00 am.
Why do you have to catch a train that early from Madrid? I would come to Madrid the prior evening. You do understand how far Toledo is from Madrid?
I agree with Frank. If you have to be in Barcelona that early, come to Madrid the night before. If you don't, take a morning train from Toledo to Atocha, then a train from Atocha to Barcelona. When I did this in September 2015, my schedule was:
Depart Toledo 9:25 Arrive Madrid Atocha 9:58
Depart Madrid Atocha 10:30 Arrive Barcelona Sants 13:15
Remember that because of the Spanish meal schedule, that put me in Barcelona in time for lunch.

I had to buy this as two separate tickets, since for some reason the Renfe website would not show this as one itinerary (I found the trains on Bahn, which showed all the options). And if you do this, know that trains from Toledo arrive at Atocha's lower level, while trains to Barcelona leave from Atocha's upper level. You don't have to leave the secure area to change levels; ask for the elevator between them (it's outside near the high number tracks).

We have an apartment in Toledo booked through Airbnb for two nights. Unfortunately, we haven't been able to get a lot of information from our hosts regarding bus/train/shuttle timetables. Our flight out of Spain is the following day. The hope is to spend our last day relaxing in Sitges during the afternoon/evening. Catching a late morning train from Madrid - Barcelona makes this difficult as getting from Barcelona - Sitges is another 30-40 minutes. Hence our interest in getting up early to catch an early train. As an aside, getting up that early won't phase us much as we're a bunch of early birds anyways.

Do I understand you correctly that you want to go from Toledo to the Barcelona airport for a departure back to the US? That is fairly risky if correct.
Frank, I think they're going to Barcelona in the morning, planning to spend most of the rest of that day at Sitges, then flying back home the following day.

Keep in mind that the first train out of Toledo (to Madrid), is at 6:25am but only on Monday- Friday, and at 9:25am on the weekends ( and on November 1st for some reason)

acraven has it right: the plan is to spend the day in Sitges and fly back home the following day. I would be too stressed taking a 3+ hour train in order to catch an international flight.
And I realized the timetable mistake after having posted earlier. Unfortunately we're traveling on Sunday, so the early morning train options aren't available to us. Hence my initial post. I was scratching my head as to how I couldn't find a 6:25/6:50/7:25 Toledo departure before...
If you're set on leaving on the 6:20am train from Madrid, I suppose you could have your hotel book a taxi for you.
