We're thinking about 4-5 days in Basque country in the middle of October and would like recommendations for must-see/do, places to stay (both location and hotel), eat, etc. I am also thinking we may want a guide/driver for 3 or so out of the 5 days. Recommendations, or suggestions how to find and engage someone good? What would we expect in terms of fee structure for the latter?

I've never used a private driver/guide. The Basque Country is an affluent part of Spain, so it will not be cheap. I got to all the above places except Lekeitio via buses and trains in 2016, but I had more time than you will.

That's not much time. Plan for two days each in Bilbao and San Sebastian. I really enjoyed seeing Guernica as a day trip, but if you're not a fan of Picasso's masterpiece, you could skip it. Also consider the French side: St-Jean-de-Luz, Biarritz, or the hill town of St-Jean-Pied-de-Port.
