For our October visit, I managed to secure the Alhambra ticket at the very last minute, with the last resort (Granada Card).
I was wondering if there is anything else with such short supply, and also worth visiting in Madrid, Toledo, Segovia and Seville.

The Alhambra is the most difficult by far in all of Spain. In Madrid and Sevilla you may avoid waiting in line for tickets to the top sights (Prado and Alcazar, respectively) but they don't sell out. And it may be wise to buy tickets to the Palace in Madrid a couple days in advance or a flamenco performance in Sevilla.
Lines for the Alcazar and Cathedral in Seville were very long in April. Rick explains how to avoid the line at the Cathedral in his book. I was able to buy the Alcazar ticket a couple of days in advance, but one section (the Private Apartments??) sells out much, much earlier. If you're interested in those, check on tickets right away.
