Need passenger locator forms for each country when traveling through Europe?

We're leaving for Madrid the end of March and traveling through France, Italy, Austria, Germany, Brussels, back to France and leaving out of Frankfurt the end of May. Traveling via rental car.

Do I need to fill out the EU passenger locator form or any other country-specific passenger locator forms other than upon arrival in Spain?

Are there any potential issues with Customs (either entering Spain or traveling around Europe) with us visiting so many countries? I assume we won't run into Customs on the highway at the country borders? We're triple vaxxed and have our CDC cards.

You only need the EU passenger locater forms for France, Italy, Malta, and Slovenia. See: https://app.euplf.eu/#/
You will need to fill one out each time you visit one of these countries. (So, twice for France)
Regarding Customs: once you've legally entered the EU there is no border or customs check between countries. That said, you could be stopped to show your CDC vaccination card (booster required to be considered "fully vaccinated.") Also, know that each country has its own Covid regulations. In Italy, for example, a FFP2/N95 mask must be worn everywhere---indoors AND outside. No cloth or surgical masks are accepted. So, make sure you understand the regs, be prudent, and have a great trip!

Basically you need to look up the information for each country and fulfill those requirements. I know taking the train into Germany from Belgium, the conductor asked about a German form, essentially a Passenger Locator Form, that he gave us the link to, that we had to fill out.

By rental car, I doubt anyone will know or check, it is just an obligation you have, to obey any requirements...not sure if there are even any repercussions if you should miss something.
