Just something to keep in mind, but there are a number of conferences taking place in Madrid throughout October and they can have a huge impact on hotel prices. On a couple of our trips hotel prices were double to triple the normal rate.

We liked the H10 Villa de la Reina. it's right on the gran via, has wonderful staff, breakfast is available. In the morning there is free coffee in the lobby, and free cava in the evenings.
We stayed at Hotel L'Opera. Near (ha ha) the National Theater. Nice room, decent breakfast. We could walk almost everywhere.
We are booked at Central Palace Madrid for this fall. It's a 3 star property with 21 rooms, about $300 a night (our 3 night stay is $958), close to all the attractions. It does not include breakfast but does offer it for about 10euros. I spent a lot of time looking for something like what you are looking for, and it's the best I could come up with at that price point. It's rated pretty highly.
whitestavern -- thank you for the suggestion. The Central Palace sounds wonderful, but reviews mention the noise as an issue. Did you find the same?
I did see a couple of reviews mentioning the noise, but I also read several that noted how quiet it was. Tripadvisor calls the hotel style quiet, and the rooms are described as soundproofed. If you like everything else about it, I would just reserve an interior facing room, which should greatly reduce the outside noise. I'm a sucker for a view so booked an exterior room, but I will bring earbuds just in case.
We’ve stayed at Apartosuites Jardines de Sabatini on 6 occasions. Large rooms with kitchenettes. The breakfast is excellent, but we mostly have breakfast at Café de la Riviere, almost next door.
We stay in the superior studio rooms with views of the royal palace. It’s a family run boutique hotel with an extraordinary classic car museum under the hotel.
I was just going to post that we just got back from Madrid and stayed at the lovely Hotel Preciados - a block from Callao metro, three blocks from Sol metro, very, very central, charming, big rooms. Four star. Wonderful people at the front desk.. Can't ask for a better location. Incredibly comfortable (and large) bed. Large bathroom with nice shower. We paid about $265 a night, including tax. No breakfast, but free minibar (with water, beer and juice).
I did just look on their website quickly for October, though, and it's much more expensive that month. November cheaper again, in case you are not committed to that month (why does it go up so much that month, I wonder?).

Just discovered another hotel, and I may be switching since it is $229 less expensive than Central Palace Madrid and includes breakfast, which gets high marks. It is Hotel Francisco I. Here's the link: https://www.hotelfrancisco.com/en/
