Guidance in Filling Out the Spain SpTH form

Hi,

I need guidance from travelers who have been to Spain recently and had to fill out the SpTH form. I’ll be traveling from the US to Barcelona with a 2-hr layover in Lisbon.

  1. For the flight information, do I enter the flight number of the Lisbon to Barcelona flight?
  2. For the country of origin of the flight, do I enter Portugal then?

Thanks in advance for your help!

I have similar concerns, on the Spain health site it says children under 12 or transiting through the airport don't need the form completed, but I have a short connection departure for US from Bologna to Madrid, arriving in Charlotte NC. I can't take the chance if the if the Madrid connection on Iberia doesn't allow the transiting without the code.
I have created an account on the Spanish health site.
But not sure how easy it is to update my vaccine proof, not sure if I can get the code, not sure if I need a paper QR code or just share on my phone, etc

I think we have a different situation. I’m transiting in Lisbon, Spain as my destination. Yours is the other way around, as I understand. You’re transiting in Madrid with Bologna as the destination.

I would enter the flight number for the Lisbon -Barcelona flight, but use U.S. as country of origin. If using the app ensure it doesn’t change dates in you. I filled mine out using the website.

Once you get your QR code, take a screenshot of it and save it to photos in the event you can’t bring up your mail at the desk. I also had a printed copy. Not sure about Barcelona, but in Madrid you showed the code right after passing through immigration.

I think that if transiting through Madrid Airport I don't need the Spain health code for my vaccination record, but I have a short connection from Bologna to Madrid. Anyone able to verifly this ? Personal experience? I depart on 20 September .

I transited through MAD on Friday Schengen to non-Schengen and there was no health or vaccination status check of any sort. Upon arrival (from a Schengen country) we deplaned into the secure area, so no additional security screening either.
