As much as I have read about entry requirements into Spain, I want to understand who checks or do they check your booster record. It appears Spain doesn't and you could be asked to take a covid test. Is it the airline that checks in the US when you board?
The airline should have you upload your CDC card to its system if you do online check in. Either that or it might ask to see it when you check in at the counter.
Edited: as Scudder stated, the health form is no longer required as of 20 September, so disregard what Renee stated.
You have to go online and fill out a health form 48 hours before your arrival in Spain. You will then be emailed a QR code which you need to enter the country. There is info on the Spanish website that states how long ago your last shot must be.
Spain no longer has a process for attestation- their health department app/website for the QR code system has been shut down.
Technically, the vaccine/booster or test requirement stands. I don’t know if airlines are checking this, or if it’s only an honor system with random checks on arrival.

But masking is still mandatory in all Spanish public transportation, probably taxi, too.
Just returned from Spain today after spending a month there. There was no checking on the vaccination card at airport when we landed. Masks are required on all public transportation (trains, buses, and taxis).
