Car Rental In Spain (Madrid)

I have only rented a car one other time in Europe (Sicily) and the RS Italy Forum was incredibly helpful in this experience. I can't seem to find similar advice on the Spain forum. What advice would you have for a newbie renter in Spain? I already have an IDP and plan on paying with my AmEx Platinum.
A few specific questions I have:

1) Is the AmEx Platinum insurance "enough" or should I buy additional? In Sicily they had various insurances that were super confusing (tire insurance additional, for example). Is it similar in Spain?

2) I will be picking up the car on my way out of Madrid, going towards Extremadura. To avoid city driving in Madrid, what pick up location would you recommend?

3) Any companies I should avoid (or prefer)?

Since you have an AMEX card, you could get their Premium Rental Car Insurance. The way it works is that you sign up for it. It kicks in when you use your card to rent a car. There is a very low, flat rate for a period of up to 42 days. The exact cost seems to depend on your state of residence; but for us, in California, it's around $18.
I have had experience with making claims on both the Premium insurance and on what comes automatically with the card. Both were handled without hassle.

I couldn't find a huge difference between the standard AmEX protection and the Premium for purchase. It appears just higher limits of coverage with the premium? Have you found anything else?
