Most economical way? Was considering renting a car in Pisa but could rent a car anywhere and drive to Pisa after taking a train
from Barcelona to San Remo. or of course we could fly. Generally how is driving in Italy? especially through the countrysides?

The easiest, most efficient and probably cheapest way to get from Barcleona to Pisa would be a budget flight. One airline that operates on that route is Vueling, although their flights are typically in the evening. You could also have a look at RyanAir or other carriers.
While you could certainly drive from San Remo, there are a number of "caveats" to be aware of when driving in Italy. Travel by train from Barcelona to San Remo and then driving will be more expensive and take considerably more time.
