Starting to plan a trip to Southern Spain in the fall and it appears best schedules on United fly into/out of Madrid. I will be visiting a friend in Malaga who doesn't have a car. Researched train schedules but they are confusing. Flying is an option too. Any advice?

If this were my trip, I would book a flight to Malaga, with a connection, probably in Madrid. It's just easier than having to exit the airport, go to the train station and get a train to Malaga.
Connecting in Madrid is definitely an option, but there are other possibilities.
direct trains take 3 hours between Atocha and Malaga, but as noted you have to
get downtown. If you wanted to spend any time in Madrid (or Cordoba) this would
be worth considering in one direction.
just about every major European airline has flights into Malaga, so you can connect
in Frankfurt, Paris, London, etc. United has flights from Newark direct into Malaga also.
Not sure where your starting point is, but from NC (GSO) we flew to Malaga via one stop at EWR. Flights were similar in length and cost as those to Madrid.
