We will be in Lisbon from October 1 to October 4 of this year. Can anyone recommend a guide or a tour company that we pick four people up at our hotel, give us a tour of Sintra and then return us to Lisbon?

We found a guide via Trip Advisor - he is with 'WithLocals".... he seems to 'belong' to the company but self employed (if that makes sense) and you deal directly with him and not via an unknown person.
He will pick us up from our hotel , give us a full day tour to Sintra and return us to our hotel.
There are several guides you can choose from - I picked our guide based on reviews. If you google 'WithLocals' you then just follow the menu prompts for city, etc etc.

You didn't mention it but I suppose you are looking for a private tour...
A sales champion is the Sintra-Cabo da Roca- Cascais Tour.
One very recommended guide around here is Fernando Batista from Portuguese Chauffeur I found him at https://portuguesechauffeur.com/
Seems like he has been driven a lot o folks around here with a lot of good reviews.
It deserves a look.
