I have a Charles Schwab Debit card which I use only internationally. They have no fees, ATM fee reimbursement or conversion fees. I have a set amount of money in it (around $1k) which limits my exposure if hacked.
Of course, I only use Multibanco and decline cash conversion.
Never have had a problem

If you avoid the ubiquitous Euronet machines, any other cash machine with a bank's name on it should be fine.
I traveled in Portugal last month and pulled cash from machines in Lisbon and Porto. Neither charged a use fee, and I declined their offer to convert to dollars. Always choose the local currency.
I also have Charles Schwab. Last week I took out money from a cash machine. Withdrew in local currency. The amount was converted at less than 1% above the published rate.
If you make certain to only use Multibanco machines, you won’t have a problem. They always have the name on them. There are many and a lot of times close to or on the outside of banks. The Euronet machines are blue and bright yellow. Avoid them!

Besides the Schwab cards, very often good credit unions allow fee-free withdrawal from ATMs -- though the bank that owns the ATM may tap you for a fee.
Adding to prior posts, you want to look for ATMs with the words ”Multibanco” / ”MB” on them (example of symbol is on Wikipedia page)

Make sure to choose to get your currency in Euros rather than dollars. I can’t speak for Capital One, but I did not pay any fees when using the Multibancos with my home bank.
There are other ATMs such as the ones operated by Euronet which charge fees; avoid them. I ran put of cash in Sintra and got stuck using one of these and paid a fee for it.
So I tried to withdraw from Santander and it only allowed me to check my balance. Luckily, I ended up finding a multi banco nearby. It also asked me if I wanted a currency conversion to USD, I said no and then it asked me AGAIN to make sure. Haha.
