International Drivers License Portugal

When renting a carin Portugal from the big rental car company do use an international driver's license? or
just you USA state driver's license

What have you done, get an international drivers license, or just use your USA state drivers license?

Has anyone had problems with the police when they were driving on a USA driver's license only

What you would be looking for is an International Driver's PERMIT. It's not a license. It requires a visit to your local AAA office (no membership required) and for about $30 you would get the permit, which translates your license in the event that you encounter law enforcement and would present it with your USA state driver's license.

As KD stated, it is a permit not a license and is only a translation. It does NOT replace a valid driver’s license. At the rental counter you’ll be asked for you U.S. license. We always have an IDP although nobody has ever asked to see it.

Regarding being stopped by police, the chances are slim unless it’s a speed trap or you happen to be breaking some law as you pass one. You’re more likely to get caught by a speed camera. In that case, you’ll find out when contacted by the rental company.

You do not need an IDP in Portugal if you have an American or Canadian driver's license.
