I tried Guerin elsewhere in Portugal and I can recommend them, it is a reputable company.
The one company you generally want to avoid is Goldcar, anywhere in Europe: infamous for surprise extra fees.
I’ve used Guerin several times and have had zero problems. But, when I was on Madeira, I was happy I didn’t rent a car. The hills are really steep and ones I would not like to drive on, especially with a manual transmission. We managed very nicely by taking tours around the island. If you do rent one, I would suggest paying the extra for an automatic..
It's been a while, and I can't remember who we rented from, but likely to have been Hertz.
It's fun to drive round the whole island, one road goes the whole way, plus you can split it in two with the central road that goes over the central plateau, often above the cloud.
One tip - avoid a convertible - the tunnels often have unfinished rock surfaces with mini waterfalls!
