I thought we would get a car in Lisbon and drive to Sintra, stay over and then continue on a road trip through other parts of Portugal. After reading a bit more it seems driving there may not be optimal. If we bus or train in, then can we get a car there for the rest of the trip? Will this add significant expense compared to airport pick up in Lisbon.

It is possible to pick up a car in Sintra and I would definitely do that rather than driving from Lisbon. The issue might be drop off fees if you don’t want to bring it back to Sintra when you drop it off. The train from Lisbon to Sintra is easy and inexpensive and much preferred IMO.
I always use Auto Europe for car rentals and have always been pleased.
Check the rules of where vehicles are allowed in Sintra. Vehicles needed a special pass to be near the centre of the city.
Also check the cost of drop-off fees. We were in Portugal last Fall with another couple who rented a vehicle and the drop-off fees were about €300. It was more worthwhile to keep the car for two extra days, pay for parking and the gas to drive back to Lisbon then take the train back.
