Entering Poland from Czech Republic by Plane?

What are Poland's requirements for an American moving from Czechia into Poland for tourism? One detail seems to inevitably contradict another as you crawl through the fine print...

Arrivals from the Schengen Area:

1/ fully vaccinated with EMA approved vaccine:
locator form + vaccination certificate

2/ unvaccinated:
locator form + PCR or rapid antigen test done in the country of departure or within 48h on arrival in Poland. If you decide to do a test on arrival in Poland you are registered for quarantine which starts at midnight and is lifted automatically as soon as the lab enters your negative result into the Sanitary Inspectorate database. Most, if not all, airports in Poland have testing facilities but the test can be done at any lab of your choice.

I have read that Poland accepts the American CDC vaccination certificate as proof of vaccination. Is this accurate?

I'm not aware of any problems with accepting CDC cards
