Poland is huge, I would concentrate on large cities like Warsaw (and others). Go online to their visitor webpages and you'll see many options, like the Praga district in Warsaw across the river. Also check out In Your Pocket Guides for each city and just find the art/ museum sections there. There is a wide variety of art in the major cities - all the options you listed.

I really enjoyed the poster museum in Warsaw. Really good graphic art there. However, the museum is temporarily closed for work on the building, and that could take quite some time, I imagine.
