Is the paper CDC COVID VACCINE CARD acceptable to enter the Netherlands or is a QR code required?

We are planning to visit the Netherlands beginning 4/21. I know pertinent info can and likely will change by then, but I can't tell if we have to present a QR code proving vaccination (including booster shot) to be admitted to the Netherlands, or if our CDC card will be acceptable to enter the country and gain access to all places there. If we take a COVID test the day before we fly out, on 4/20, will that be within an acceptable time frame or will it be considered to soon to count?
Thanks to anyone who has solid knowledge. I've read and reread the government website and have been unable to determine what the government requires.
