We, 3 adults, are flying in to Milan Oct 5th 2016, and driving to Montacentini Terme for 5 nights. There we are exploring Tuscany. Then we will drive to Amalfi for 5 nights. We have 2 nights to do something before we fly home, 4pm flight, from Milan on the 18th. Any suggestions? Thank you in advance!

I would spend both nights in Stresa, on lake Maggiore, only 45 min drive from the Milan Malpensa airport.
Actually I would even take one night from Amalfi and add it to Stresa. Four nights are probably sufficient for the Amalfi coast.
Lake Maggiore and nearby lake Orta (especially the village of Orta San Giulio) have quite a bit to offer, therefore you probably need more time, two nights is only one day.
