We have been to Rome before and toured the Vatican but would like to return to visit only the Sistine Chapel and not the museums, Basilica, etc. How do we arrange or plan for that?

The only way to get to the Sistine Chapel is through the Vatican Museums (unless you are the Pope, or an Archbishop attending a Conclave in the Chapel, etc.!). Thus, you will need to enter the Vatican Museums and walk through them on the direct route to the Chapel. There are many posts on this forum discussing various tours that would get you early entry, including some tours that go straight through to the Sistine Chapel in order to arrive there before the crowds get there.
