I did a Chianti tour with Italy and Wine last fall and would recommend them. Departed from Florence, visited a large winery in the morning and a smaller organic one in the afternoon, with a stop for lunch. Cost was 135 euro per person. As I type this I am looking at a small jar of honey on my kitchen counter that I bought at the farm store at the second stop, Millefiori. It was a delightful day.

if you do Montepulciano you aren't that far from the fabulous Brunello's over in Montalcino. Well worth a visit to a winery near each town. Biondi Santi for Brunello and Salcheto for Montepulciano are my favorites
If by needing transport you mean you won't have a car, try Tours by Roberto. Rick Steves' uses him and our experience with him was exceptional.
Viator has a fantastic winery tour that does Montalcino, Pienza, and Montepulciano for less than $100 per person. That was our favorite day of our Italy trip last fall.
