Vatican reservations - what info to fax?

Hello, I went to the Vatican site and can see the number for the fax but what information do I need to include? I was thinking names, date and time, and response fax # but I thought there would be a form or something to complete. Also, do you pay when you show up for your tour?

I just did this a few weeks ago. The info to provide is correct, but give a second choice for date/time if you have some flexibility. The confirmation fax indicates you pay, in cash (euros)when you pick up the tickets.
