I just looked at the official Vatican website ticket purchase and there was no option that included lunch. It sounds like you are looking at some other site. There's a cafeteria and a few vending machines. I've brought sandwiches and eaten them in outdoor areas. I seem to remember an outdoor cafe in the large courtyard as well. Caveat - it's been a few years since I last visited.
Thanks, after your choose the date and time for the tickets, scrolling down further on the same ticketing page they mention a lunch option with a date and time to choose. I assume that this is the restaurant, not the cafeteria.
If I remember correctly, there is really only a cafeteria, not a restaurant with table service. If I missed something about an actual full-service restaurant in our previous visits, I am interested to find out about it.
We found the "cafeteria" to be entirely satisfactory, and well located for a return to the art galleries after a break. There's also a bit of sun and fresh air available. It is unrealistic to imagine exiting the Museums for a two-hour slow-food meal of hand-crafted Italian delicacies. (Even if re-entry were allowed, which is inconceivable, you'd have an hour in the security line again ... )
