They are getting stricter with the dress code for the Sistine chapel so cover those shoulders and knees.

I reserved online, it was well worth the small fee to skip the long line. Be sure to print out your reservation and take it with you. There is a separate (very short) security line for reservations.
I used Ron in Rome's directions to get to the Vatican. They were spot on - also his tips for where to go once you are inside.
Well, do you want to do a tour ?
We had a 3 hr tour that also allowed us to get in through the tour entrance and avoid the lines. The tour was great and informative and we had a small group. We used Presto Tours, and they have an online site and you can checkout tripadviser for reviews about them.
