If you're willing to hit the ground very early in the morning, and if tix are still available, you might want to consider the early entry package. There was one that included a breakfast buffet (American style! But hey, at least you get juice and coffee to fuel you through the morning) and then you get entrance to the museum before the general opening time. You get a head start to see the museum with very few other visitors, and you get to the Chapel before it is full of people. We were among the very first people to walk into the Chapel when we did this package. (The second time I went with general entry, it was the predictable mob scene!)

We just came back from Rome and did visit the Vatican & Sistine Chapel. We booked through Walks tour. Our guide was fantastic. My advice is to book the earliest entry possible. We felt our “early entry” 7:15 was not early enough due to so manny tours happening. By time we wrapped up our time (11am) Saint Peter’s was getting very crowded. Definitely go, it was a huge highlight for us. There is so much to see so when you book, make sure your ticket covers what you want to see. We especially loved the map room. Enjoy your time.

These days there are no regular commercial tours with access to the Vatican Museums before the public entry time, which I believe is now 8 AM. There may be some very pricey special arrangements for early or after-hours tours.

I thought same but checked into the Walks of Italy website and it looks like they again offer a tour that enters before 8:00 if you can land it. Reference their advertised 6:45 AM tour slot? That said, they state that the 6:45 tour (and 6:30 PM + 7:30 PM tours) do not include the basilica. That can be sort of a big deal if looking to book specifically for that benefit.
