We bought Underground and Belvedere tours in Spanish

Our family of 3 are going to Italy for the first time at the end of August and I am beyond excited! Coopculture Colosseum U&B tours in English were sold out so we signed up for the Spanish one. Both my husband and I speak some Spanish, having taken a couple of years in college and having traveled several times around Mexico, so we should be able to comprehend at least the basics of the tour, but our 14 yo son does not. Do the guides talk to you directly or through headphones? Do you think it would be ok for him to just not wear the headphones as I think I'd get a headache if someone was talking right in my ear for over an hour in a language I could not understand? We could just discretely explain things to him.
