I'm planning a trip in October 2023 based on Rick Steve"s Tuscany Itinerary.
Day 13 says Sleep In Lucca and Day 14 says Fly Home.
I'm assuming that means fly home from Pisa Airport. What is the best most efficient way to get from
Lucca to Pisa airport.
You can take a train from Lucca to Pisa Centrale and then the People Mover to the airport. 46 minutes, less than 10 € pp.
Trains can be either the Best or the worst way, it depends on your flight's departure time and destination.
By the current winter schedules, the first train to Pisa Centrale departs from Lucca at 5:35 AM
It's ~30 minute car ride.
Door to door service and not having to drag your bags on/off trains may be 'best'.
Perhaps the hotel is able to give you an estimate of taxi cost.
Day 12: Tour Over After Breakfast
Breakfast is provided, but there are no group activities today. It's a breeze to reach the Florence or Pisa airports by train. Your guide will help you with any post-tour planning, leaving you well prepared for the road ahead.
Where the flight goes, (Schengen vs. non-Schengen) will affect how early you need to be at the airport
As Dario says, it depends on when your flight departs and what you're comfortable with.
We requested that our host transport us to Pisa airport. With hindsight, it was expensive and a "white knuckle" ride, but we certainly got there on time!
If you are headed to the States from Lucca you will need to connect somewhere else in Europe and the choices are very limited - Paris Orly; Frankfurt or London. Florence would also require a connection but you have a wider choice of airlines, flight times and connecting airports. For direct flights from Italy to the US you need Rome or Milan. If you are doing the trip with a rental car it’s about a 3 hour drive to Milan.
Rather than slavishly following the RS itinerary, you could look at ways to adjust your itinerary to suit your needs.
Don’t use a group travel itinerary as a tour is very different from traveling independently. A bus isn’t going to pull up to your hotel to take you to a specific location on your itinerary. Get a general idea but do not “copy” it.
If you find a flight that works either from Pisa or Florence you’ll need to spend the night before in your departure city. Those flights will leave quite early in am, not direct to US.

RS tour itineraries are very fast paced and rely on tour bus etc
That one in particular would need a rental car if on your own
And is Elba really a priority to you?
