We are planning a fall trip to Italy likely based in Sienna for a few weeks and considering a second base elsewhere in Tuscany or northern/central Italy. I’d love to hear some suggestions - visit will be in October and/or November. I like Lucca but have already been there several times.

Not sure the size of the town you prefer.
I think Montepulciano, Certaldo, or San Gimignano are great small towns. I prefer smaller vibe.

We love Siena and autumn is a great time to visit. You didn't mention if you would have a car, but if you do, you can visit many of the small towns of Tuscany while staying in Siena.
If you have not stayed in Bologna/Modena/Parma in Emilia Romano, you might want to consider one of those cities for your second location. Bologna and Parma are great food cities and each has an interesting history worthy of exploration. These cities are close together and travel between them is very easy by train.

Thank you for the great ideas! We are thinking Montepulciano might be fun as the Christmas market starts in late November. Also enjoyed Bologna on an earlier trip but didn’t have time for smaller towns in the vicinity.
Consider visiting nearby Umbria and staying in Spoleto. Visit Perugia, Assisi, Gubbio and Orvieto.
A car will be necessary in most locations outside the provincial capitals that are well served by public intercity transportation.
If you have a car you can stay anywhere, just be aware that the end of October/early November is the rainiest time of the year.
