Trip cancelation, trip interruption , medical insurance. Should we consider for independent travel? If any products better than others?

I always get travel insurance, primarily to cover medical and evacuation/expatriation. As for trip cancellation and interruption, it's a personal thing. Can you afford to lose the money you have already put out? I compare policies at insuremytrip.com and buy the one best suited for that trip.
Trip cancellation insurance is full of loop holes, restrictions, and expensive so we never get it. At this stage of the game we basically self insurance since we have never had any problems. However, med insurance is a completely different issue. You need to know what your current insurance covers and doesn't and fill in the gaps. Second we have medical evac insurance in the event of a really bad situation. That type of insurance is cheap and worth it.
We have used Travel Guard and been pleased with the peace of mind.
