Another option you could consider would be to use a budget flight from Pisa Airport. Although I hesitate to suggest it, RyanAir appears to have a few direct flights (1H:30M). Other airlines also operate on that route, but appear to have one layover.

Travel time from Manarola to Pisa Airport is as short as about 1H:25M with one or two changes, depending on which train you choose.

You could also travel from Milan / MXP Terminal 2 to Catania using easyJet as they have numerous flights each day, starting as low as about 36.49€ (plus a few fees and taxes of course). I've found that the flights with them are so much more comfortable if one adds Speedy Boarding and some of the other extras. The trip from Manarola to MXP T2 will take about 5 hours.

Genova Airport is another option, but the flight choices don't appear to be as frequent.

Thank you for the suggestions. How much time should we allow for the connection to get from Genoa or Pisa train station to airport?

If you are going to Sicily from the Cinque Terre, it makes sense to fly from either GOA or PSA.
The Cinque Terre are located in between.
From Genova Piazza Principe station to GOA airport, it's a 30 min bus shuttle ride. You can save a few minutes by taxi, but it will cost more.

From Pisa Centrale station to the airport the distance is so short that there is a people mover connecting the two. You need just a handful of minutes to get there.

Whichever airport you decide it has the most convenient flight for you, try to be at the check-in at least one hour prior to flight departure.
