I am going to Italy next year and I need to take my migraine meds with me...they need to be kept cold. What is the customs procedure for this.

I travel with meds that need refrigeration all the time. If my hotel doesn’t offer an in-room refrigerator, I ask reception to put it in their refrigerator. Never had a problem. If you’ll be traveling around by train, just keeping it cool for long periods of time might take another step - get the hotel to freeze your ice packs.
A s Christine said, Customs doesn't care, but Security does. You can use refreezable freezer packs (they will stay frozen longer than gel packs), and put everything in an insulated container like a fabric lunch box. That should last for most point to point travel. In between, as suggested, ask your hotel to store the pack in their freezer if you don't have a fridge with freezer section.
Alternatively, look at the Frio insulated insulin cooling cases. No ice pack required.
Do be aware the Italian authorities may have rules that are different than US TSA rules, so for the return tip you may have a different experience.
