Not sure where you are seeing 6 hours. Almost every train connection I checked between Lucca to Milan is right about 4 hours, give or take a few minutes.

Lucca to Milan is generally just about 4 hours by train, with a connection usually in Florence, or sometimes another connecting station. According to Rome2Rio.com, by car the trip is just under 3 hours, but that likely depends on traffic. Train fare is very reasonable, especially if you can book the Florence to Milan trip well in advance and at a discount, as I did for October 2013. Renting a car just for that trip would no doubt be considerably more expensive, unless you already have the car for others parts of your trip, or if perhaps there are several people in your group. Arranging for a car and driver for that 3 hour trip would likely be the most expensive, quite expensive actually, especially assuming the car and driver would need to make an empty run in one direction.
