Here's how travel by train works in Europe:
You don't need to decide the route the train takes
what you do is go to this train schedule site
You specify the starting and ending points, and Trenitalia will take it from there and present you with a schedule of trains for a given day, and they decide the route, not you.
Once on the website:
input starting and ending points
ending point: you'll have to select one, the mid-lake towns are the ones most commonly visited by people here, Varenna, Bellagio or Menaggio are popular
for date just enter an arbitrary date that coincides with weekday or weekend, whichever is applicable
input 0600 to see all schedules for the day
it will show you the schedules
You don't need to decide now. When you get to Italy, stop by a train station and they'll help you. It's not possible to buy tickets online in advance with a US-issued credit card.
There is really only one routing. Monterosso to Milan and then Milan to Lake Como. As Kent suggests, use the linked train schedule to enter your date and time of travel. Your train from Milan to Lake Como will be a local Regionale.
We just took the Regionale train from Milano Centrale to Varenna-Esino and back this past weekend. The ride's short and scenic. Be sure to purchase your return ticket at the same time you buy the outgoing (or at least in advance). We took an evening train back to Milan on Monday night and the Varenna station was closed for all intents and purposes. There was a sign on the door saying that no tickets could be purchased there; and something about visiting a travel agency down in Varenna for tickets. I wasn't worried since we had tickets but if we didn't we'd have been in trouble. Also, apparently the yellow boxes where you validate your tickets are not operational if the station isn't open. Neither of the boxes worked at Varenna and appeared not to be "on". Nobody asked to see our ticket though so no harm, no foul.
