Tuscany is roughly equivalent to a state in the USA (although, generally a lot smaller). Inside, Tuscany, you will also find sub-regions such as Chianti.
The capitol of Tuscany is Florence (English name)/Firenze (Italian name). It is a 1 1/2 to 2 hour train ride from downtown Rome to downtown Florence.
Florence is also the defacto Tourist Center of Tuscany. It is a major transportation hub with regional train and bus lines spreading out into the surrounding countryside. One easy self-guided day trip from Florence is to Pisa and Lucca...
If you would rather take an escorted trip into the Tuscan countryside, there are hundreds or coach (big bus) and small-group tours focusing on many different interests.
I view having a car as being a nuisance in Italy if you are spending the majority of your time in the cities. The public transportation network is so good you really don't need a car unless you plan to spend most of your time in the countryside staying in agriturismos (farmhouse B&Bs).

If you intend to visit the Tuscan countryside, a car is the way to go. For the price of one tour into the hills, you can rent a car for 2 days, and visit 4-5 villages. Roads are well marked, parking is easy, and you're not tied to someone else's schedule of overcrowded tourist traps.
