We will be going to the Jazz festival in Modena in July.
I believe the show should end around midnight. I starts at 7:30, we figure it should last 4 hours.
We are trying to figure out how to get back to Bologna at that time. Any advice?

It looks like the trains from Modena to Bologna run until midnight - or 23:41 on Sundays. If you're in Piazza Roma the train station is only a 10-15 minute walk from there. I'd be sure you have the Trenitalia app and buy a ticket for the next train and start walking over there no later than 11:40 whether the concert is done or not to be sure you get back to Bologna. There train to Bologna is about 30 minutes so you'll be back before 1:00am even if you take the last train.
The locals mentioned the upcoming Jazz festival when we were there and seemed excited about it.

I tried to find the train station that we would take to Modena after the show, and the latest I could find On July 16th is 10:57.
I think I am correct.
Is there one station? The show is at Piazza Roma
There is also a train at 12:03 am (7/17)

Yes, there is only one train station and if you are on Piazza Roma facing the palace it is essentially directly in front of you behind the giant Ducal Palace building. I don't know what kind of traffic control may be set up for the concerts but the most direct way is a short walk on Via Belle Arti and then Via Sgarzeria nearly all the way to the station. The clearest route is probably to take Corso Vittorio Emanuele II which is the giant two sided traffic street directly behind the Palace to big roundabout and then head left to the station.
Just use Google maps if you have any doubts. Google has it listed as a 13 minute walk.

In Modena in fact there are other train stations, but the mostly are for local suburban traffic. Modena Centrale is the one where the trains to Bologna leave. Is exactly north of Piazza Roma, like 10 minutes walking. Via Sgarzeria is the most direct route, but is a little easier for people who don't know the city take Viale Vittorio Emanuele.
I noted on Trenitalia website that there are even trains to Bologna at 23:00 and 23:41, but probably sold out. Try to check again in some weeks: maybe they added seats or someone become available at that time.
I'd simply take the Trenitalia-Tper Regionale #2485 that departs 5 minutes after midnight (4.5 € one way). On Friday 17/7 and Saturday 18/7 trenitalia has added the Freccia #9561 at 00:13.
The Regionale can't sell out, the Freccia is 4 minutes faster.
The two night trains that depart from Modena at 23:08 and 23:41 are not sold out, the warning on trenitalia.com reads "temporily" not available, not "sold out". Trenitalia has blocked the sales only between the 13th and the 17th of July, the Jazz Festival ends on the 18th. Most probably they are looking for more sleeper cars, as it often happens with night trains. I'd check again on June 15.
