We are a group of 4 arriving at around 7:00pm at Malpensa. After a long flight from the US, we want the best option for getting to our hotel. Hotel Berna is close to the central rail station. Any suggestions for an affordable and dependable transport service that will deliver us directly to the hotel? Can we get a taxi at the airport when we arrive or do we need to pre-arrange? I understand it should be a standard fee to take us into Milan central? We'd rather not take the train or bus due to the length of the ride.

Would a taxi be any faster than a train or a bus? The fixed taxi fare is 110 EUR. A car service would be in that range.

The bus and the train take about the same amount of time, just under an hour, and I see no reason to expect a car is going to be significantly faster. A group of 4 will likely need a mini-van and it debatable if it more comfortable than a bus.
It will drop you at the door, vs having to walk across the street from the train station( which is where the bus goes),so it does have that advantage.
Your $$, your choice.

They show €140 for a minivan. But I have seen others for less (€120 for up to 6 pax).
In terms of time, chances are the travel time is similar between car, bus or train. You are looking at about 50 min on average. Bus or car may be longer than the train if there is traffic.
The cost of the train is 13€ per person, the bus is 10€ per person. They all drop you at the Centrale station. So the cost should be around 40€ to 52€ for the four of you.
The advantage of the private car is that it saves you the walk from the terminal to the airport station (across the terminal) and the walk from the Centrale station to the hotel (10 min or less, about 1/4 of a mile), but the cost will be at least 3 times as much as the bus.
