I read a post from 2010 that said the cost is about the same for a taxi, from Rome city center to the airport, than taking the train. We have 4 adults traveling. We're staying in the city center. Our flight from Rome to Charlotte, NC is at 11:15am. Experienced traveler feedback please and thank you!!
Have your hotel call a taxi (or a minivan) to accomodate all 4 of you.

I paid 45 euro in September 2014 to the airport from the center. If you divide that by 4 people it's about the same price as the train. I usually take a cab/car to airport just for convenience of pickup at the hotel and not walk to the train station.
The fixed rate was raised to 48 euro a couple of years back. You need to be in Terminal 5 around 8, 8.30 so an hour for the taxi at that time of day so you should be leaving your hotel around 7.

I was just in Rome on Monday and had to leave for the airport at 4:30 am. We were staying right near Piazza Navona. I booked a car with rometransferstyle.net and the driver was there waiting right on time with a nice Mercedes van which was more comfortable than a cab. It cost 50 euros which is only 2 more than the rate for a cab. I booked it over the internet and paid the driver once we got to the airport. It worked out great. Good luck!
I've used Claudio from neorental.it for transport. He was prompt, polite and the ride and car were comfortable. He was recommended by the Roman lady who taught my cooking class.
Last October, my private minivan (for 3 of us), arranged for by my Piazza Navona apartment host, was only 45 euro. At 0600 on a Sunday, it took about 45 minutes.
There are also booking services online if you want to arrange it yourself.
In comparison, the Leonardo Express is almost 15 euro and you still need to factor in the transport to termini unless you are in walking distance. If you do take the train, it's a fairly lengthy walk to get to a point where you pick up the shuttle bus to terminal 5 (where you check in for your flight).
We can not say enough good things about Rome Cabs. It is a private car service (not a cab company) but only cost about 10 Euro more than a regular taxi. We always use them and have always been very pleased. Go to romecabs.com and see what you think. We usually send an email and they answer us with a quote within 24 hours. You do have to pay cash (Euros) and the end of the journey but not in advance. The problem with having the hotel make arrangements is that you might wind up with a regular cab, a car service, or the desk clerk's brother in law. Happened to us once but there were four of us and only one brother in law so there wasn't a problem.
